NOTE: This form will be filled-in and executed only by the owner of record/Homeowner and Buyer. Lead-based Paint Disclosure (if necessary) –signed, initialed, and dated by Buyer. A Copy of the State Local Seller Disclosure package (as completed by the Owner of Record/Homeowner) –Signed, initialed, and dated by Buyer. A SIRVA -stamped copy of the State Seller Disclosure form -- Signed, initialed, and dated by Buyer. NOTE: SIRVA WILL PROVIDE A STATE SELLER DISCLOSURE FORM WHICH SHALL BE BLANK AND STAMPED WITH SIRVA'S DISCLAIMER LANGUAGE. SIRVA SHALL NOT FILL IN AS TO OR DISCLOSE ANY KNOWLEDGE OF THE PROPERTY Closing Agent Selection Agreement- Please have signed and dated by Buyer. Please make sure any changes (as permitted below) are initialed by Buyer. SIRVA DOES NOT REQUIRE BUYER TO PURCHASE TITLE INSURANCE FROM THE PROPOSED TITLE AND/OR ESCROW AND CLOSING OFFICE IDENTIFIED IN THE CLOSING AGENT SELECTION AGREEMENT. ANY PRE-POPULATED TERMS CONTAINED IN THE AGREEMENT MAY BE CHANGED BEFORE FINAL EXECUTION OF THE CONTRACT OFFER. The following additional documents will also be required, prior to closing: NOTICE OF COMPLETION AND ACCEPTANCE OF REPAIRS. (To be executed AFTER WALKTHRU and BEFORE CLOSING. Any verbal agreements otherwise from any source or party shall not alter this requirement.)
